If you have an appointment at Charles Retina Institute and would like to register in advance, you may print the registration form PDF below, complete all four pages, and bring the forms with you to your appointment.

What to Expect on Appointment Day
Expect your appointment to take between two to four hours. This is due to the need to have your eyes dilated, possibly get retinal pictures and imaging studies, as well as any treatments you may require in addition to seeing your physician for consultation.
What to Bring
Please remember to bring your insurance card(s), a picture ID, a referral (if required) and your contact lenses and/or glasses. We also ask that you bring all of your current medications (including over-the-counter medicines) in their original containers or a detailed list (medicine, strength, dosage, usage).

Medical Record Requests
